VF Training Volleyball
Participant Waiver, Release of Liability,
Medical Authorization & Program Policies

Assumption of Risk

I voluntarily assume all known and unknown risks associated with volleyball participation, including falls, collisions, sprains, fractures, concussions, illness, permanent disability, paralysis, and death.

​

Voluntary Participation

Participation is voluntary and I certify my child is physically able to participate.

​

Release of Liability

I release VF Training Volleyball, its owners, coaches, contractors, volunteers, facility owners, school district and affiliates from liability to the fullest extent permitted by Florida law.

​

Emergency Medical Authorization

If I cannot be reached, I authorize emergency medical treatment and accept responsibility for medical costs.

​

Health Certification

My child is healthy to participate and will not attend while sick with a contagious illness.

​

Medication & Allergies

Parents are responsible for medications and informing coaches of allergies or medical conditions.

​

Parent Responsibility

Parents are responsible for timely drop-off and pick-up. Supervision exists only during scheduled practice times.

​

Personal Property

VF Training Volleyball is not responsible for lost, stolen or damaged personal property.

​

Attendance & Refund Policy

Tuition is non-refundable. No refunds, credits or make-up sessions for missed practices or cancellations due to facility issues, weather or events beyond VF Training Volleyball's control.

​

Athlete Conduct

Unsafe or disruptive behavior may result in dismissal without refund.

​

Parent Conduct

Parents and spectators must behave respectfully or may be removed from the facility.

​

Weather / Force Majeure

Practices may be modified or canceled due to weather, emergencies or other uncontrollable events.

​

Equipment

Athletes must use equipment properly and follow coach instructions.

​

Communicable Diseases

Participation may expose athletes to communicable diseases including COVID-19 and influenza.

​

Independent Contractors

Some coaches may serve as independent contractors.

​

Governing Law

Florida law governs this Agreement. Venue shall be Orange County, Florida.

​

Severability

If one provision is unenforceable, the remainder remains effective.

​

Electronic Signature

Electronic signatures have the same legal effect as handwritten signatures.

​

Transportation

No transportation is provided unless separately authorized.

​

Communication Consent

I consent to receive communications by email, phone, text and WhatsApp.

​

Injury Reporting

Injuries should be reported immediately to a coach.

​

Dress Code

Athletic clothing, athletic shoes and no cell phones or jewelry during participation.

​

Coaching Decisions

Team placement and coaching decisions are at the discretion of the coaching staff.

​

Photo & Video Release

I authorize the use of photos/videos of my child for marketing and promotional purposes without compensation.

​

Entire Agreement

This document represents the entire agreement.

​

Acknowledgment

I have read, understood and voluntarily signed this Agreement.
